La il·lustració de Dalí va ser reproduïda en heliogravat amb remarques a la punta seca a partir d’un dibuix a ploma amb tinta xinesa. L’artista també va dissenyar la vinyeta que es va reproduir tant a la coberta com a la portada del llibre. Els textos, estructurats en quatre seccions –L’Homme (L’home), Les Possessions (Les possessions), Les Médiations (Les mediacions) i Le Jugement Original (El judici original)–, van ser concebuts amb la voluntat de trencar amb les fórmules literàries tradicionals i estan inspirats en la simulació de deliris. El text L’Homme s’havia publicat un mes abans a Le Surréalisme au Service de la Révolution, n. 2, l’octubre de 1930, acompanyat d’altres vinyetes realitzades per Dalí a petició de Breton i Éluard, que van advertir l’artista –tot disculpant-se prèviament– de la necessitat de moderar el contingut de les seves il·lustracions per evitar que la revista fos retirada per “obscenitat” per part dels censors (1). Dalí també va redactar el full de mà (Prière d’insérer) del llibre, en el qual afirmava: “Si el Primer i el Segon Manifest van ser la declaració del contingut manifest del somni surrealista, LA IMMACULADA CONCEPCIÓ és l’exposició del seu contingut latent” (2). El frontispici de Dalí al·ludeix de manera literal a un dels aforismes inclosos a Le Jugement Original.
